Online services is a quick and easy way to:

  • book, check or cancel appointments with a GP or nurse
  • order repeat prescriptions
  • see parts of your health record, including information about medicines, vaccinations and test results
  • see communications between your GP surgery and other services, such as hospitals

How do I get access to my Online Services?

To access online services, you will need to complete an Online Access Request form and attend one of our surgeries with proof of identity.  One of our team will then add the access to your records and you will receive an email linkage pin which you will need to enter on the nhs app or website.

I could see my records from birth at my old practice, why can’t I see them now?

Our practice policy allows access to patient records depending on when our patients register with the practice.  We are responsible for ensuring that we do not share any third-party information or information which may not be appropriate / safe for online visibility.  Unfortunately, we are unable to confirm that your previous practice/s have removed this information from online visibility so cannot approve you access to this.

We have provided our patients access to their online records since November 2021, however this is not the case for all other practices with online access only becoming mandatory from the 1st November 2023. 

We have outlined the online access approval dates in the table below.

 

I want access to my full GP record; how can I do this?

Every patient has the right to view their full GP record.  If you wish to request your full record you can do so by completing a Subject Access Request form (SAR). Once we have received your request, your records will be reviewed, and any third-party information will be redacted (removed from view). Your full records can then be sent to you via email or added to view on the nhs app.

This process can take up to 4 weeks to complete.

I am a carer / family member and require online access to act on a patient’s behalf.  How do I do this?

Accessing health services on behalf of someone else is called Proxy Access (or Linked Profile in the nhs app).  This can enable a parent, carer, or family member to access certain health services for someone else from their own online services account.

 

Accessing for another adult (over 16)

To request proxy access, the patient will need to complete a consent form and come into the surgery to hand this into to our reception team along with proof of identification. Once this has been approved, your access will be added.

Access for another adult is given based on them requesting it themselves as they understand and agree to it. Adults aged 16 and over are assumed to be able to understand and consent to their own treatment.  This is called having capacity.

If the person you want to help cannot understand or agree to you acting for them, for example because of a condition such as dementia, this is called lacking capacity.  In this instance we may be able to approve access without consent, however this will need to be discussed and agreed by a clinician.

Proxy access for children and young people - Under 16

A parent or person with parental responsibility can request proxy access for a child under the age of 11 at any time.  In some circumstances you may be asked to provide proof of identification or proof of parental responsibility.

When the child us coming up to their 11th birthday you will be notified that your access will be restricted, and the patient will be required to consent to this being reinstated. The patient will be required to complete the consent form and attend the surgery in person.

Children have the same legal rights over their data as adults.  Children aged 11 and over are usually considered to have capacity to consent, or refuse access, unless for example they have a medical condition or learning disability that affects their understanding.

When a person turns 14, proxy access is removed, and the patient will be required to complete a further consent form and present to the surgery with proof of identity.
